Cytochrome P450 family 2 subfamily C member 19 (CYP2C19) is an enzyme related to drug metabolism, which is primarily expressed in the liver. Clinically, medications significantly influenced by CYP2C19 activity include proton pump inhibitors and the antiplatelet agent clopidogrel (marketed as Plavix).


Impact of CYP2C19 variants

Specific single nucleotide polymorphisms (SNPs) in the CYP2C19 gene may affect enzyme activity. CYP2C19*2 and CYP2C19*3 are common reduced-function alleles that reduce CYP2C19 activity and may influence drug metabolism. Clopidogrel requires CYP2C19-mediated activation to exert its antiplatelet effect. Reduced enzyme activity may decrease active metabolite formation and platelet inhibition. In addition, CYP2C19 intermediate and poor metabolizers may show reduced clopidogrel response and increased risk of cardiovascular or cerebrovascular events.

CYP2C19 genetic variations affect drug activation and metabolism, leading to differences in drug response among individuals. Asian populations show the highest prevalence of CYP2C19*2 and CYP2C19*3 alleles among major ethnic groups, with frequencies of approximately 36.6% and 7.5%, respectively.
